Patient treatment plans contained “recycled language and generic information” despite differences in background, diagnoses and behavioral goals. Activities that were supposed to prepare patients to live independently instead involved little interaction, with some patients sitting by themselves or wandering. And plans to help patients transition back into the community or residential facilities were often incomplete.
